Registered Nurse Care Coordinator - Part-Time Home Care

What you'll do

• Serve as a primary nursing contact for clients and families
• respond to inquiries during assigned 7 a.m.–7 p.m. on-duty hours within 30 minutes
• limit communication outside those hours to emergencies
• conduct initial consultations and 90-day reassessments, including nursing assessments and care plans
• supervise and mentor CNA/GNA caregivers
• attend monthly company meetings
• coordinate care and resolve client or family concerns.
