Client & Family Support Specialist — Bilingual English/Spanish

The Client & Family Support Specialist role is in person in Santa Rosa, CA, with a Monday–Friday schedule from 8:30 AM to 5:00 PM. Compensation is $25–$27 per hour. Pacific Health Group’s Client and Family Support Program helps families and caregivers of Full Service Partnership clients navigate behavioral health and community services. The specialist serves as a non-clinical guide and advocate, helping families access care, community resources, and support. Through education, outreach, and coordination with FSP teams, the role contributes to family engagement and more effective navigation of health and social service systems.

What you'll do

• Coordinate with families, clients, and FSP teams to develop family-inclusive service plans
• guide access to behavioral health services
• connect families to housing, food, and employment resources
• provide information and support related to rights and grievance processes
• host educational workshops and resource clinics
• conduct community outreach
• connect families with support groups
• document encounters, referrals, and service plans
• record attendance and gather workshop feedback.

Why this role
