The Jail Navigator Monitor position is onsite in Cape Girardeau, MO, with a Monday–Friday schedule from 8:00 a.m. to 4:30 p.m. and pay of $19.87 per hour. The role supports inmates at Cape Girardeau County Jail and is funded by a 13-month Missouri Department of Public Safety grant. The monitor connects incarcerated adults with health and community services during incarceration and after release. By coordinating referrals, transportation, benefits, and continuity of care, the role supports re-entry and the program’s efforts to reduce recidivism and use of force.

What you'll do

• Track program and grant needs
• review and follow up on mental health screening results
• coordinate or provide release transportation
• connect inmates with community resources and treatment
• arrange provider appointments
• coordinate medication continuity with jail medical and psychiatric staff
• communicate with the Department of Corrections about transfers
• assist with benefits applications
• maintain resource packets and required documentation
• provide case-management-type services
• participate in an on-call rotation when necessary and eligible.
