The Social Worker role is an onsite position in San Leandro, CA, with home visits and travel to hospitals and other locations to meet participants and service providers. Pay is $37.97–$56.95 per hour. Center for Elders' Independence is a PACE organization using an interdisciplinary team to provide integrated care and support participant autonomy and community living. The social worker assesses participants' psychosocial needs, develops and implements care plans, and facilitates communication among participants, families, and staff. The role contributes to coordinated care for frail and elderly participants through case management, referrals, family conferences, and collaboration with the interdisciplinary team.

What you'll do

• Develop and implement integrated plans of care as an interdisciplinary team member
• monitor progress and communicate participant changes
• participate in behavioral management meetings and psychosocial rounds
• complete assessments, reassessments, and care plan updates
• maintain timely chart notes and case files
• refer participants to community resources
• investigate and report elder abuse complaints
• conduct conferences and annual reviews
• maintain agency relationships
• make home and hospital visits.

Why this role

Master's degree in social work required; one year working with frail or elderly people preferred. Current CPR and First Aid, California driver's license, acceptable driving record, and reliable transportation required.
