What you'll do

• Build referral relationships with social workers, case managers, discharge planners, senior living leaders, and community partners
• identify families who may need private-pay care after hospitalization, rehabilitation, surgery, or functional decline
• educate referral sources and families about care options
• conduct outreach and follow-up
• attend networking events, community meetings, and partner visits
• manage long-cycle opportunities
• and track activity, lead sources, referral outcomes, and client conversion.
