Community Support Supervisor

The Community Support Supervisor role is onsite in Los Angeles, CA. Compensation is $29 to $32.50 per hour. The role supports Community Support services for health plan members at Serene Health, a division of Optima Medical Management Group, whose mission includes promoting wellness and long-term well-being. The supervisor guides Community Support staff and oversees reporting, assessments, referrals, authorizations, and housing deposit documentation. The work helps ensure services are coordinated accurately, consistently, and in line with health plan requirements and applicable processes.

What you'll do

• Provide leadership and support to Community Support staff
• generate and analyze member progress reports
• maintain the Master Log
• review analytics, productivity logs, and member notes
• oversee staff notes and authorizations
• coordinate with health plans on referrals and authorizations
• monitor assessment quality and regulatory adherence
• coordinate housing forms and deposits
• maintain documentation and respond to stakeholder inquiries.
