The Director of Social Services role is onsite in Cambridge, MA. Compensation is $105,000–$110,000 per year. The Neville Center at Fresh Pond provides short-term rehabilitation, long-term care, and skilled nursing services. The director leads and monitors the center’s social work services to support residents’ psychosocial wellness and care transitions. The role coordinates assessments, care planning, discharge planning, and community resources so residents and families can participate in decisions and access support for safe community reintegration.

What you'll do

• Direct and monitor social work services
• promote residents’ psychosocial wellness
• participate in resident assessments and discharge planning
• maintain and communicate the discharge calendar
• manage the MOLST function with admissions
• coordinate care-planning meetings
• develop discharge plans with residents and address barriers to community reintegration
• coordinate with community resources
• discuss advance directives with residents and families
• complete assessments
• serve as ACO liaison and coordinate case-management tasks
• facilitate 72-hour interdisciplinary meetings.
