The Perinatal Navigator role is onsite at Legacy Acres Homes in Houston, TX, with compensation not specified. The position works Monday–Friday, either 8 a.m.–4:30 p.m. or 8:30 a.m.–5 p.m., supporting pregnancy, postpartum, and early-childhood care navigation. The role exists to help pregnant individuals and families access care, education, and community resources, reduce barriers, and support maternal and infant health outcomes. It works with an interdisciplinary clinic and care team on pregnancy testing, appointment coordination, patient engagement, and continuity across maternity and pediatric care.
What you'll do
- Conduct point-of-care HIV and pregnancy testing and health screenings
- schedule appointments and follow-ups
- conduct outreach
- provide culturally sensitive prenatal, nutrition, and healthy-pregnancy education
- screen patient needs and support goal setting
- implement retention strategies for OB and pediatric patients
- connect clients with community resources
- document services in the electronic health record
- collaborate with the interdisciplinary care team
- meet applicable performance and grant requirements
- participate in meetings and training, including travel to different Legacy clinics.
