What you'll do

• Provide skilled home-based care
• perform comprehensive assessments and OASIS documentation
• develop and adjust individualized care plans
• monitor patient conditions and report changes to physicians
• educate patients and caregivers on medications, treatments, disease management, and home safety
• coordinate with therapists, aides, and other providers
• prepare patients and families for discharge
• document care in the EMR.
