Patient Navigator — Breast Health

What you'll do

• Review and process new-patient referrals
• interpret clinical documentation and route referrals
• identify urgent or high-risk cases
• communicate with referring providers
• schedule consultations, follow-ups, imaging, and diagnostics
• explain care steps to patients
• communicate imaging results and next steps
• document in the EHR
• scribe patient encounters and assist with chart completion.
