What you'll do

• Monitor resident health and changes in condition
• provide physical care within LPN or RN scope
• administer medications and treatments as prescribed
• receive and execute physician orders
• implement and evaluate care plans
• document care and incidents in electronic health records
• communicate with families and physicians
• direct and coach caregivers
• support behavioral interventions and memory care practices
• respond to resident and family concerns.
