The role contributes to coordinated home health care by monitoring progress, educating patients and caregivers, documenting services, and collaborating with the therapy team.

What you'll do

• Implement treatment plans developed by the supervising Speech-Language Pathologist
• provide speech and language therapy in patients’ homes
• assist with communication, cognitive, and swallowing skills as directed
• monitor, document, and report patient progress and condition changes
• educate patients and caregivers on therapy techniques, home exercises, and communication strategies
• maintain patient documentation
• collaborate with the therapy team and other healthcare professionals.
