What you'll do
- Conduct initial and ongoing comprehensive assessments
- determine eligibility and appropriate levels of care
- develop, implement, and document plans of care
- educate patients and families on self-care, nutrition, and dietary needs
- determine service scope and frequency
- assess caregiver ability to meet patient needs
- provide preventive and rehabilitative nursing procedures
- apply infection-control precautions
- direct home health aides and LPN/LVNs
- prepare clinical and progress notes
- report changes and outcomes to physicians and other personnel
- monitor cases for third-party payer requirements
- communicate with patients, families, and care professionals.

Why this role
