Intensive Care Coordinator – Bilingual Spanish or Portuguese

What you'll do

• Complete progress notes and home-based behavioral health assessments, including age-appropriate CANS
• maintain a caseload of 11–14 families
• assemble Care Planning Teams and guide individual care and safety plans
• monitor service quality
• connect families with services and resources
• arrange transportation and crisis support when needed
• collaborate with providers and identify natural supports.
