What you'll do

• Support marketing, communications, branding, and image
• attend relevant network meetings and expos
• establish and maintain contacts with social workers, discharge personnel, physicians’ offices, nursing homes, and other referral sources
• facilitate educational opportunities
• build relationships with facilities and community organizations
• educate healthcare professionals, patients, and families about home care services
• coordinate patient admissions
• conduct onsite visits to assess needs and barriers
• maintain records of interactions and referrals in CRM
• respond to customer complaints.
