The SAGE program serves adults with severe and persistent mental illnesses; DESC provides behavioral health services and supportive housing for people experiencing homelessness and complex health conditions. The psychotherapist provides short-term individual and group therapy within a multidisciplinary team. Through counseling, assessment, outreach, crisis response, and coordination with care partners, the role supports clients’ recovery goals and engagement with services.

What you'll do

  • Provide brief individual counseling using cognitive behavioral therapy within a recovery model
  • facilitate group counseling
  • develop measurable therapy goals
  • document services
  • collaborate with care team members and assist with individualized service plans
  • conduct outreach and engagement, psychosocial assessments, and Less Restrictive Alternative reviews as needed
  • provide crisis intervention and risk assessment
  • support clients in maintaining healthy living conditions.
