What you'll do

• Coordinate patient and caregiver schedules
• manage open shifts, call-outs, cancellations, missed visits, and reassignments
• participate in a rotating after-hours and weekend on-call schedule and respond to urgent coordination needs while on call
• perform electronic visit verification (EVV) and visit verification
• communicate with patients, caregivers, RN supervisors, and office staff
• resolve scheduling issues and concerns
• maintain accurate scheduling and patient information
• follow home-care documentation, privacy, and compliance requirements.
