The role supports timely, coordinated transitions from admission through post-discharge care.

What you'll do

• Work with the interdisciplinary team to guide treatment plans
• coordinate tentative discharge and contingency plans
• participate in discharge planning
• facilitate weekly team conferences and treatment plan changes
• complete required documentation
• review case management reports
• address payor requirements and recertification needs
• meet with patients and families within stated timeframes
• arrange referrals and resources
• and participate in the case management on-call schedule as needed.
