Social Worker (MSW)

Assess patient's and family's psychosocial needs related to terminal illness. Consult and collaborate with the interdisciplinary team in developing and implementing the comprehensive care plan. Document problems, psychosocial assessment, appropriate goals, care provided, interventions, and patient, family, and caregiver response. Assess and refer for ongoing bereavement services.
