What you'll do

• Monitor and supervise care for members requiring complex observations
• review referrals and assess needs during visits
• assign appropriate staff
• develop and revise care plans
• supervise aides and LPNs
• complete and review clinical records
• communicate with members, caregivers, providers, and care coordinators
• conduct face-to-face home visits
• arrange staff orientation and training
• and educate members and caregivers about health needs.
