The Healthcare Outreach Specialist role is onsite in Columbus, OH; BoldAge PACE provides an all-inclusive, personalized care program intended to help older adults live independently at home. The specialist builds community and healthcare referral relationships, educates prospective participants about PACE, and supports the path from referral to enrollment. The role contributes to program growth while helping older adults and caregivers understand services suited to their needs.

What you'll do

• Develop and execute outreach plans aligned with growth goals
• identify and cultivate referrals from healthcare providers and community organizations
• conduct consultative conversations with prospects and caregivers
• document outreach and prospect activity in the CRM
• maintain referral-partner relationships
• represent the program at community events and presentations
• use demographic and market information to guide outreach
• coordinate with enrollment, benefits, assessment, and interdisciplinary staff
• work toward referral, enrollment, and outreach goals
• maintain confidentiality and follow safety and infection-control protocols.

Why this role
