What you'll do

• Develop and deliver activity and recreational programming
• complete activity assessments and documentation, including relevant MDS and care-plan sections
• attend resident care plan meetings
• prepare and post monthly activity calendars
• identify resident preferences and barriers to participation
• develop individualized plans
• support group and individual leisure activities
• plan community activities involving residents, families, staff, and community members
• maintain physician approvals and resident precautions.
