What you'll do

• Develop and maintain relationships with physicians, hospitals, clinics, nursing facilities, assisted living communities, rehabilitation facilities, and other referral sources.
• Conduct face-to-face visits and educate partners about services, eligibility, benefits, and referral processes.
• Follow up on referrals while protecting patient confidentiality.
• Identify referral opportunities, track outreach and referral activity, participate in community events and educational programs, and coordinate with clinical and intake teams.
• Follow HIPAA, organizational policies, Medicare requirements, and applicable regulations.
