What you'll do

• Assist with support-service activities, independent living training, management plans, and supported-living assessments
• carry out skills training plans
• assist with personal care, mobility, and communication
• develop natural and community supports
• assist with medical appointments and health needs
• document services and help prepare participant reports
• attend staff meetings and communicate with supervisors.
