Systems Navigator — Exceptional Family Member Program

What you'll do

• Conduct intake interviews and document needs
• assess family-member needs
• provide referrals, follow-up information, and advocacy
• collaborate with civilian and military providers
• conduct home visits as needed or requested
• coordinate respite-resource information, monthly support groups or workshops, and program briefings
• maintain resource catalogs and records under Army and DoD regulations
• facilitate warm handoffs
• collect data and complete monthly reports
• implement practices connecting families to support services.

Why this role

Requires a bachelor’s degree in a listed human-services, counseling, education, or public-health field and two years of family-related case-management or social-work experience.
