The Certified Nursing Assistant (CNA), Home Care role is onsite in Grand Junction, Colorado, with a schedule of four 10-hour shifts. Pay is $18.50–$22.13 per hour. HopeWest’s PACE program provides coordinated care to help older adults maintain health and live safely at home. Working under a licensed nurse and an assigned plan of care, the CNA provides personal care and support, assists with daily activities, promotes safety and comfort, and reports changes in participants’ conditions to help preserve independence and dignity.

What you'll do

• Provide personal care and supportive services in participants’ homes
• assist with activities of daily living under a licensed nurse’s direction and the assigned plan of care
• promote comfort and safety
• observe and report changes in condition
• maintain accurate documentation and follow care plans.
