What you'll do

• Lead and oversee an interdisciplinary team
• ensure adherence to hospice regulations and standards
• facilitate communication between medical staff and the care team
• lead interdisciplinary meetings and document patient needs and care plans
• develop and update care policies and emergency procedures
• manage staff scheduling and performance reviews
• coordinate communication with families, physicians, and providers
• analyze quality metrics and manage a caseload as needed
• participate in after-hours and on-call duties.
