The Behavioral Health Support Specialist role is onsite in Warsaw, NY, with services delivered in office-based, community-based, and telehealth settings according to program needs and client preferences. The role supports engagement and continuity of care through person-centered services, coordination with providers, outreach, and accurate documentation.

What you'll do

• Deliver individual and group psychiatric rehabilitation interventions
• provide targeted case management, coordination, referrals, advocacy, and follow-up
• orient new clients to services and rights
• use motivational interviewing and person-centered planning
• coordinate with treatment teams and higher levels of care
• conduct outreach after missed appointments
• maintain provider communication, required consents, community resource information, and compliant electronic health records
• participate in meetings, supervision, and training.
