Designated Coordinator — Home and Community-Based Services

The role works with individuals, direct support staff, case managers, healthcare professionals, legal guardians, and other partners to coordinate services and document outcomes.

What you'll do

• Coordinate and document implementation of each individual’s Coordinated Services and Supports Plan Addendum
• coordinate with case managers, lead agencies, healthcare professionals, and legal guardians
• develop person-centered support plans
• monitor and document service delivery for Minnesota 245D compliance
• participate in admissions and service delivery
• conduct home visits when required
• train and oversee direct support staff
• review outcomes
• report maltreatment, incidents, and injuries
• maintain documentation
• protect individual rights
• support audits and corrective actions
• provide direct care as needed.
