What you'll do

• Follow care plans and assist with activities of daily living
• observe, record, and report changes in clients’ conditions
• document care and visit time
• communicate with clients, families, nurses, and agency staff
• assist with homemaking, medication assistance, and wound care according to the care plan
• attend agency meetings and in-service programs.
