What you'll do

• Lead infection prevention and control duties
• conduct acute-care and long-term-care surveillance and report to NHSN
• track data and diseases, monitor outbreaks, and conduct long-term-care outbreak testing
• maintain chronic care management for eligible patients
• provide monthly foot and nail care at rural health clinics
• provide health and wellness education and counseling
• support annual wellness exams and advanced care planning
• maintain confidential employee health records and vaccination records
• coordinate return-to-work protocols
• support quality improvement
• reconcile medication lists and collaborate on care plans.
