What you'll do

• Provide school-based case management and care coordination
• build supportive relationships with students and families
• assist with assessments and person- or family-centered treatment planning alongside licensed clinical staff
• identify behavioral, social, emotional, medical, and educational needs
• connect families with services and resources
• coordinate with therapists, schools, providers, and agencies
• advocate for students and families
• conduct outreach and follow-up
• complete screenings, intakes, referrals, and documentation
• monitor progress and communicate with the treatment team
• participate in school and community activities.
