Clinical Care Leader (CCL) - Full-Time Days

Utilize the nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation) to provide individualized nursing care in the home setting. Collaborate with inter-professional colleagues to plan, implement, and evaluate care. Document resident history, symptoms, medications, and care provided. Utilize technology to enhance quality resident care.
