What you'll do

• Integrate and coordinate social-service and discharge-planning activities with the facility team
• develop measurable plans and objectives
• conduct psychosocial assessments and reassessments
• identify and address resident and family psychosocial needs
• promote residents’ rights
• involve residents and families in care planning
• provide individual and group support during adjustment, crisis, or need
• coordinate and direct department activities
• support social-service and clinical staff
• maintain working relationships with medical staff and other departments.
