ECM Care Coordinator

Job overview

The ECM Care Coordinator role is primarily in-office in San Fernando, CA, with required field visits to patients in the community. • Conduct patient assessments and develop individualized care plans

• conduct quality-assurance reviews and collect program data
• perform field visits to establish ongoing eligibility
• monitor patient safety and implementation of the Individual Service Plan
• address patient concerns and advocate for resources.
