Intake Counselor

What you'll do

• Serve as first contact for prospective participants and families
• conduct preliminary needs assessments and participate in intake interviews
• compile intake documentation
• coordinate orientation
• represent Pace to referral sources
• coordinate aftercare and transition services
• monitor and document progress
• provide career counseling and job-placement assistance
• participate in family conferences and care reviews
• plan support groups and coordinate interventions.
