What you'll do

• Support individuals with personal basic needs and daily activities
• assist with activities of daily living and instrumental activities of daily living according to individual support plans and other support documents
• participate in developing and implementing support plans
• communicate with individuals and families
• document daily activities and goals using a smart device
• maintain confidentiality and complete required state-mandated training
• assist with lifting or transferring individuals as needed.
