What you'll do

• Assess patients’ home-care needs
• develop and revise individualized plans of care
• determine nursing diagnoses and interventions
• administer ordered medications and treatments
• supervise LVNs and CHHAs under agency policies
• educate patients and families
• coordinate with care providers and physicians
• document clinical notes
• ensure appropriate medications and durable medical equipment
• and participate in QAPI, training, and monthly in-services.
