What you'll do
- Provide benefit enrollment assistance
- Assist with redetermination requirements
- Assist with enrollment in supplemental food programs
- Partner with patients to ensure healthcare funding
- Schedule and attend appointments with SSA, DDS, and OHS
- Gather and submit financial information
- Maintain client caseload.
