What you'll do

• Receive calls from field staff and physicians about client care issues and orders
• enter appropriate orders and update plans of care, medication profiles, and schedules
• document calls in coordination notes
• notify staff to support care coordination
• review and approve interim orders before physician submission
• support intake and process referrals as needed
• assign licensed professionals to evaluation visits
• participate in case conferences as needed
• process recertification and discharge decisions based on clinical documentation and case manager input
• follow up on order tracking and entitlement verification
• verify orders for requested medical supplies.
