What you'll do

• Complete ongoing physical and psychosocial assessments
• develop and revise plans of care with the interdisciplinary group, patient, family, and attending physician
• provide education and interventions for pain and other symptoms
• ensure necessary equipment, supplies, drugs, and services are available
• document services accurately and attend interdisciplinary group meetings.
