What you'll do

• Follow care plans in collaboration with the Director of Nursing, clients, caregivers, physicians, and other care team members
• conduct aide orientations, quarterly supervision visits, skill checks, and annual performance activities
• instruct aides during field visits
• review personal care plans and orient aides
• document services and changes in client conditions or home situations
• communicate concerns to case managers or legal representatives and families.
