Serves as designated divisional liaison with providers and ministry Care Coordination teams, to move patients towards safe and effective discharge plans or transitions to the most appropriate next level of care. Plans and develops reporting tools for sharing of information between division and ministry teams. Fosters collaborative working relationships within and beyond the Care Coordination teams
