What you'll do

• Provide in-home patient care
• work with physicians and oversee LPNs, CNAs, and aides
• examine and assess patients, document progress, and report changes to physicians
• educate patients and families about care plans, medications, therapy, diet, exercise, and home safety
• coordinate care goals and discharge from services.
