Community-Based Hospice Registered Nurse (RN) Case Manager

The Community-Based Hospice Registered Nurse (RN) Case Manager role is field-based in Wilson and Nash Counties, NC, with a Monday–Friday schedule from 8:00 a.m. to 5:00 p.m. Compensation is not specified. Transitions LifeCare provides compassionate care and support to individuals and families facing serious illness and end-of-life care. The nurse leads patient-centered hospice care by assessing needs, managing symptoms, coordinating an interdisciplinary team, and supporting patients and families. The role also maintains compliant documentation, contributes to quality initiatives, and mentors new team members.

What you'll do

• Develop and manage individualized hospice care plans
• conduct nursing assessments and interventions
• monitor conditions and adjust plans
• coordinate interdisciplinary care
• delegate and supervise LPNs and hospice aides
• educate patients and families
• complete clinical documentation
• monitor medications, supplies, and equipment
• participate in quality improvement
• assist with onboarding and mentoring.
