The Primary Care Physician, Managed Care / Value-Based Care role is an onsite position in Brandon, FL, with compensation of $300,000–$412,000 per year. The senior-focused practice provides relationship-based primary care for adults aged 65 and older, with smaller patient panels, longer visits, and clinical support. The physician provides comprehensive care to Medicare Advantage and senior patients, including preventive care, chronic disease management, and acute concerns. The role supports patient outcomes by developing individualized care plans, coordinating with care teams, closing care gaps, educating patients and caregivers, and maintaining timely electronic medical record documentation.

What you'll do

• Provide primary care to Medicare Advantage and senior patients aged 65 and older
• perform assessments and develop individualized care plans
• manage chronic conditions, preventive care, and acute medical concerns
• deliver patient-centered care in a managed care/value-based care model
• coordinate treatment with physicians, specialists, and interdisciplinary teams
• identify and close care gaps
• educate patients and caregivers
• build relationships with patients and families
• maintain accurate and timely electronic medical record documentation.
