What you'll do

  • Meet with assigned individuals at least monthly to monitor requested services
  • lead annual Person Directed Plan meetings
  • coordinate annual CFC meetings with Medicaid provider representatives
  • complete assessments and state documents
  • coordinate with providers and state officials
  • connect individuals with community resources
  • respond to crisis situations
  • travel within the Texas Panhandle's top 26 counties.
