Mental Health Outreach Counselor

The Mental Health Outreach Counselor is a full-time, onsite role in Greenfield, MA, with pay of $18–$20 per hour. The position is part of ServiceNet’s Mental Health Recovery Division outreach service. ServiceNet supports people facing mental illness, developmental disabilities, brain injuries, homelessness, and substance use issues. The counselor helps participants pursue individualized recovery goals and supports daily living skills, safe community living, and access to services. The role works with a multidisciplinary team to develop and review treatment plans and document participant support.

What you'll do

• Assist participants in varied settings with recovery goals
• collaborate on treatment plans and review goals and tasks weekly
• teach daily living skills related to housing, self-care, work, education, social supports, finances, transportation, community resources, and medication independence
• monitor safety in community living
• provide transportation as assigned
• complete required documentation
• attend team meetings and trainings
• assist residential programs as needed.
