Job overview

The Tailored Care Management Care Manager role is onsite in Wilson, NC. Compensation is $21–$28 per hour. The position supports North Carolina DHHS Tailored Plan care management for members with behavioral health, intellectual and developmental disability, or traumatic brain injury needs. The Care Manager coordinates services and supports across the care continuum, working with members, families, caregivers, and care teams to pursue quality outcomes. The role includes connecting people with community resources, supporting care plans, and providing crisis response for recipients receiving the service.

What you'll do

• Provide professional services and treatment within the scope of NC DHHS Tailored Plan Care Management
• under supervision, provide or ensure education on self-advocacy, connect members and caregivers to training and services, explain rights and protections, promote wellness and prevention, connect members to employment and community resources as appropriate, support family-driven strategies, develop and implement crisis plans with the care plan or ISP, provide first-responder crisis response on a 24/7/365 basis for recipients receiving the service, respond face-to-face within two hours, and complete requested community-resource referrals.

Why this role

Requires a related bachelor's degree or RN licensure, plus two years of relevant direct experience. NCI and Community First Aid/CPR certifications must be maintained. Crisis response is required for recipients receiving the service, including face-to-face response capacity within two hours.
