Hospice Community Manager

The Hospice Community Manager role is an onsite field position serving the Lakes Region, including the Greater Laconia, Meredith, and Wolfeboro areas of New Hampshire. The annual salary range is $80,000–$95,000. The role supports Granite VNA’s Community Hospice Program. The manager oversees clinical operations and nursing staff to help coordinate high-quality hospice care in the community. The role supports patient care planning, staff development, regulatory compliance, and coordinated work across the interdisciplinary team.

What you'll do

• Supervise community hospice nursing staff and assigned employees
• collaborate with the Hospice Medical Director and providers on patient plans of care
• support care coordination and clinical needs
• oversee staff orientation, scheduling, hiring participation, competencies, and performance reviews
• participate in quality improvement, medical reviews, surveys, and case conferences
• monitor documentation and compliance
• participate in the Administrator on Call rotation
• travel throughout the service area for patient visits and meetings.

Why this role

Requires a bachelor’s degree in nursing, three to four years of recent nursing experience including two years in hospice or palliative care, and two to four years of supervisory experience. Off-hour, weekend, and holiday work may be required.
